The Patient Portal is a way for you to communicate online with your community health center.
Registered patients will have secure and confidential access to the website and may:
- Request an appointment
- Request a medication refill
- Ask your medical team a question
- Receive test results and information online

To get started with the Patient Portal, visit your health center and ask for an account to be created.
All you need is your email address. We will create a NextGen account for you and give you a personal enrollment token (an 8-digit number used to sign in).
Once we create your account, you will receive an email with easy instructions on how to enroll. That's all there is to it!
